Damage Control Thoracotomy: A Systematic Review of Techniques and Outcomes

Abstract Background : Damage control surgery is the practice of delaying definitive management traumatic injuries by controlling hemorrhage in operating room and restoring normal physiology intensive care unit prior to therapy. Presently, damage or “abbreviated” laparotomy used extensively for abdominal trauma an unstable patient. The application a approach thoracic less established there paucity literature supporting refuting this practice. We aimed systematically review current data on thoracotomy (DCT), identify gaps techniques temporary closure. Methods An electronic search Pubmed, MEDLINE, Cochrane Database Collected Reviews from 1972-2018 was performed using keywords “thoracic,” “damage control,” “thoracotomy.” Studies were included if they reported use DCT following survival as outcome. Results Of 723 studies, seven met inclusion criteria total 130 operations. Gauze packing with closure skin suture most frequently form overall rate studies 67%. Survival rates ranged 42-77%. Average injury severity score 30, 64% penetrating nature. common complications infections (57%; pneumonia, empyema, wound infection, bacteremia), respiratory failure (21%), ARDS (8%), renal (18%). Conclusion may be associated improved critically injured patient population. Delaying operation temporarily closing thorax order allow time restore considered strategy impact open chest has remains inconclusive well best mechanisms Multi-center are required elucidate these important questions.
